Diagnostic Imaging

There must be a structured curriculum to train the resident in Diagnostic Imaging. At a minimum this shall include:

  • Didactic and clinical experiences.
  • Utilization of appropriate radiographic studies.

ACGME

The curriculum should include diagnostic imaging interpretation and nuclear medicine therapy pertinent to family medicine.


Description of Rotation or Educational Experience

Diagnostic Imaging and Nuclear medicine is an integral part of most of our educational experiences.

The program teaches Diagnostic imaging and Nuclear Medicine in various ways:

  1. There are structured didactic conferences that are presented by the FM faculty and local radiologists.
  2. Regular x-ray rounds are held on the inpatient Family Medicine Service FMS.
  3. The residents are required to read all x-rays obtained on their patients with the help of the supervising attending faculty.
  4. Radiographic studies are also reviewed with the residents on most of the clinical rotations including ER, musculoskeletal, sports medicine, pediatrics, surgery, maternity care, neurology and others.

The hospital has a digital x-ray system so that all studies may be viewed from any computer in the hospital with the installed software. This has made it possible for FM faculty to review with the residents all radiographs, ultrasounds, CTs, MRIs, and Nuclear Medicine Studies of FMS patients during rounds daily and in the FMC when precepting. The general and interventional radiologists are readily available for consultation and teaching and an elective rotation in radiology is available.

GOALS

The goal of the Diagnostic Radiology educational content is to provide learning opportunities that will enable residents to develop or refine the knowledge, skills, and attitudes necessary to:

  • Understand the indications for specific imaging tests ordered by family physicians.
  • Use imaging services efficiently and appropriately.
  • Comprehend reports provided by radiologists.
  • Independently interpret images read by family physicians.

Expectations:

Residents

It is expected that all radiographic studies ordered by a resident on the FMS service be reviewed with the attending. The resident must read the films to the best of his or her ability with the attending. Competence in basic CXR reading is an expectation by the completion of residency following a step by step method.

Faculty

The Family Medicine and Radiology faculty will supervise the residents in each of their patient interactions. If applicable the faculty are also responsible for completing an evaluation of the resident at the end of each rotation. All procedure will be directly supervised.

Goal

Residents must be able to provide patient care that is compassionate, appropriate, and effective for the treatment of health problems and the promotion of health.

  • Generate an appropriate differential diagnosis when reviewing a radiological study
  • Make informed decisions radiologically, about diagnostic and therapeutic interventions based on patient information and preferences, up-to-date scientific evidence, and clinical judgment
  • Use information technology to support patient care decisions and patient education with respect to their radiological exam
  • Determine the urgency of the patient’s need for radiological imaging.
  • Write clear orders for the indicated test. Avoid ordering more than or less than the optimal test
  • Plan the preparation of the patient for the examination ordered

Medical Knowledge

Goal

Residents must demonstrate knowledge of established and evolving biomedical, clinical, epidemiological, and social-behavioral sciences, as well as the application of this knowledge to patient care.

  • Demonstrate an investigatory and analytic thinking approach to clinical situations
  • Define specific terms used in reports
  • Read plain x-rays of the chest, abdomen, and extremities.
  • Diagnose fractures on plain x-ray views.
  • Identify on evidence of osteoarthritis, osteolytic, or osteoblastic lesions on x-rays.
  • Become familiar with MRI, MRA, CT, Nuclear Medicine, and Ultrasound

Practice- Based Learning and Improvement

Goal

Residents must demonstrate the ability to investigate and evaluate their care of patients, to appraise and assimilate scientific evidence, and to continuously improve patient care based on constant self-evaluation and life long learning. Residents are expected to develop skills and habits to be able to:

Goal

Residents must demonstrate interpersonal and communication skills that result in the effective exchange of information and teaming with patients, their families, and professional associates. Residents are expected to:

  • Communicate effectively with physicians, other health professionals, and health related agencies
  • Act in a consultative role to other physicians and health professionals

Objectives

  • Residents will read a chest x-ray in an organized and concise manner.

Teaching Methods

Residents will provide medical care to patients under the direct supervision of the attending physician. They will be supervised on all procedures performed. They will participate in weekly conferences. Residents will be assigned specific reading topics to be completed by the end of their month on the service.

Assessment Method (Residents)

Direct observation with a final global assessment based on the REDI system. All procedures will be recorded on New Innovations.

Assessment Method (Program Evaluation)

Residents will be provided with an evaluation of their experiences and given the opportunity to provide feedback of the rotation during their quarterly IEP assessments.

Level of Supervision

The resident is directly supervised by the attending physician during all procedures performed. An Attending will be available the entire time the resident is on service.
